The Standardized TB Nursing Case Management Tool and the evaluation forms for TB case manager’s performance

What it is:

The Standardized TB Nursing Case Management and the evaluation forms for PHN Standards of Practice in Los Angeles County are found in the Public Health Nursing Practice Manual (PHNPM).  The PHNPM serves as a template and a resource guide, to direct and define the standards of practice of pubic health nursing.

Why this is a best practice:

It provides step by step guides for the TB case manager on management of TB cases and suspects, contact investigation, and source case findings in a documented converter.

The Evaluation forms can be used by the TB case manager’s supervisor to evaluate the records of each TB case manager and summarizes outcomes of the evaluated records for individual TB case manager’s performance.

Resources required (staff, time, and tools):

TB case managers and supervisors should utilize the PHNPM TB Section C & F.  Staff advisors ensure new staffs are orientated to the manual.
