Donations We encourage donations to the travel grant fund. These donations are fully tax-deductible. By allowing greater participation in our educational conference, the travel grant program contributes to the development of excellence in tuberculosis prevention and treatment, a key feature of CTCA's mission. You may mail elect to include a donation with your registration or by mailing a check payable to CTCA Travel Grant Fund to 424 Pendleton Way, Oakland, CA 94621. Refunds As a policy we provide a refund to individuals if you requested it up to three business days in advance of the conference. For those who pay with credit card, we will refund minus the cost of transactions. After the cutoff date, if there were extenuating circumstances (illness or emergencies) we will do our best to refund the fee minus the cost of food. Individuals may have to wait up to two months for a refund.
Continuing Education Units & Continuing Medical EducationThis activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of the Francis J. Curry National Tuberculosis Center and California Tuberculosis Controllers Association. The Francis J. Curry National Tuberculosis Center is accredited by the ACCME to provide continuing medical education for physicians. This conference is approved for 7.50 continuing education hours. The Francis J. Curry National Tuberculosis Center designates this educational activity for a maximum of 7.5 AMA PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity. Certificates will be mailed four to six weeks after the course to all participants who requested CME or nursing credits and who successfully complete the course. Successful completion includes signing in each day, attending the entire course and turning in a completed evaluation form while on site. Note that to be eligible to receive CME/CEs for this conference, you must have filled out and turned in the evaluation while on site. You will only receive units for sessions you attended and evaluated. If you leave sections blank, we will assume you did not attend that session. CME/CEs will not be provided to individuals who mail their evaluation post-conference, no exceptions. |
