IMPORTANT REGISTRATION INSTRUCTIONS
 
1. In the Additional Information box on the registration page, write-in: a) any dietary restrictions and b) your top two breakout choices for May 15th. The breakout options are:
(1) NAHPF Breakout
(2) Short Course Regimen
(3) Lab Issues
(4) TB and HIV 
If you are registering more than one person, provide the names, credentials, license numbers, dietary restrictions and the top two breakout choices for each of your registrants.

2. You are fully registered when either:
a) You complete the transaction with paypal (credit cards or paypal accepted) and receive a completed transaction email from paypal or 
b) You email me to let me know that your check is on its way (judith.thigpen@lung.org).
